When to make your reservations

| (Requests for same day travel is not guaranteed and may not be available for requested time) |
Information needed — please give in the following order:
| 1. Last name, first name |
| 2. ID number |
| 3. Pick-up Address |
| 4. Destination Information (including building, specific drop-off and return info) |
| 5. Appointment time and pick-up time |
| 6. Will you be traveling with a Personal Care Attendant (PCA)? |
| 7. Will you be traveling with a companion (other than PCA)? |
| 8. Pick-up location telephone number |
| 9. Destination telephone number |
| 10. (If doctor’s appointment) Name and telephone number of physician |
Reserving Rides for Specific Times
| 1. Tell reservationist the time of your appointment |
| 2. Allow enough time to debark and get to your appointment |
| 3. Know the opening closing time of your destination, to avoid waiting outside |
| 4. Allow plenty of time to finish your appointment |
| 5. Be ready when bus arrives |

Other Reservation Tips
| 1. Ask reservationist if there is a specific ADA bus stop at your destination |
| 2. Reserving trips early will provide the best chance at a specific day and time |
| 3. Service peaks are 7 am – 10 am & 12 pm – 5 pm, off-peak times are best |
| 4. When traveling during peak times allow extra time |
Changing Reservations
| 1. Contact 815-937-4287(4BUS) as soon as possible |
| 2. Changes must be made 1 day in advance & are subject to availability |
Canceling Reservations
| 1. Contact 815-937-4287 (4BUS) as soon as possible – leave a message if closed |
| 2. Cancellations must be made 2 hours in advance |
| 3. If cancellations are made less than 2 hours in advance a bus may arrive anyway and you will be considered a “No-show” and your return trip will be canceled. If you still need a return ride you will need to reschedule. See Rider Responsibilities for more information on no-shows. |
| 4. Excessive cancellations within 2 hours of travel time may result in loss of reservation privileges. See Rider Responsibilities for more information on late cancellations. |
